As a service to the community, the Clerk of the Circuit Court of Collier County acts as an acceptance facility for the U.S. Passport Service in Miami, FL. In this capacity, The Clerk’s Office staff ensures that application forms are filled out properly in accordance with the U.S. Department of State requirements and are accompanied by the proper documentation and fees. 

New passport applications are sent daily, and the approved passport is then mailed directly to the applicant by the U.S. Passport Service. Processing time for each passport application varies depending upon the volume of passport requests at any given time. New passport applications are accepted at various locations and operating hours.

Passport renewal application forms (DS – 82) are to be filled out and mailed by the applicant to the National Passport Application Center following the instructions outlined on the form.

*Note: The number of Passport applications accepted by the Clerk’s office depends upon staff availability and the time required to complete the transaction within regular business hours

Requirements:
Get appropriate passport application and forms at the U.S Department of State website. For example a DS-11 form is needed for a first time applicant.
Note: Must be filled out in black ink and signed in the presence of accepting agent.
 

Submit one, color,  2x2 inch, recent, full face view passport photograph with a white background.

 

Provide proof of citizenship, using one of the following:
   - Certified copy of birth certificate issued by the Vital Statistics Office of the birth state with an official seal and file date
   - Prior Passport
   - Original Naturalization Papers

Present secondary identification:
   -

Driver’s License (If recent, additional ID will be required)

   - Government issued photo identification
 -

Must submit 8.5 x 11”photocopy of secondary identification (front and back)A fee per page is charged for each copy made by Clerk’s office.

 
NOTE:
Applicants, including children, MUST be present to apply for a passport
Children 1- 15 years old, both parents MUST appear and sign
Parent ID may include a driver’s license or passport
 
Submit personal check, cashier’s check or money order made out to the U.S. Department of State, along with the passport application. In addition, a fee paid via cash or check is payable to the Clerk of Circuit Courts.
 
Expedite:

For your convenience, the Clerk's Office offers Express Mail service for an additional fee for expediting your Passport application.  

Or, if you prefer, you may provide one prepaid overnight Postal envelope to send your application.  An additional Express Mail fee for the return delivery service of your passport will be included in your check made payable to the U.S. Department of State.

With your application, you must submit a personal check, cashier’s check or money order made out to the U.S. Department of State.
 
Renewal:
Must fill out a Passport Renewal form (DS-82).
 
Individuals that have in their possession a valid passport issued not more than 15 years ago must fill out a passport renewal form and mail it directly to the National Passport Processing Center following the instructions outlined on the form. 
 
Children under the age of 16 must submit a new passport application (i.e. cannot use the DS-82 form). Follow instructions provided on the U.S Department of State website.
